423 Eryhtroderma Due to Allopurinol Drug Reaction

Erythroderma is a condition of generalized erythema and scaling the skin. More than 90% body surface area are involved in this condition. This potentially life-threatening has been associated with high mortality hospitalized patients. The aim case report to discuss drug reaction as aetiology erythroderma its manifestations. We 55-year-old female patient who had erythema, pruritus, malaise edema her face since 7 days before admitted. She history hepatitis, diabetes mellitus type 2, was oral allopurinol treatment for last 2 weeks. Extensive erythematous plaques hyperkeratotic scales were observed face, neck, chest, arms, legs, back, gluteal. Laboratory workup revealed peripheral blood eosinophilia, mild hypoalbuminemia, hyperglycaemia, elevated AST level. got wet dressing by NaCl 0,9% bis die (BID), dexamethasone intravenously ter (TID), topical steroid BID, VIP albumin. glucose levels normalized insulin therapy, hyperkeratoric scale decreased within ten hospitalization. discharged given metilprednisolone 4 mg TID orally cefixime 200 BID orally. can be primary condition, when cause unknown, or secondary caused known diseases. One reactions. In case, because suspected due allopurinol. Protein loss hypoalbuminemia related desquamation plasma volume increased which might hemodilution severe edema. may emerges weeks several months after exposure causative drugs. presented receiving conclude that most common need careful assessment manifestation effect patient’s quality life.
